Call Stipend Submissions Blocks 1 – 7

PAR-BC is unable to pay residents submitting call after the 30 day deadline set by the LOU Commitee. Please download the form to submit any calls that were not submitted from previous blocks. These forms will be saved and reviewed by the Committee at the next meeting. Completed forms must include a schedule and can be emailed to .
