Resident Appointment Package

UBC sends out Resident Appointment Packages for all new and returning residents after the CaRMS match starting in mid-April. If you have not received your appointment package by mid-June please contact UBC.

The appointment package for new residents will include:

For your information:

  • A checklist and instructions for completing and submitting forms
  • A booklet including guidelines for residents teaching MD undergraduate students
  • Information on the Post Graduate Medical Education Registration Fee Policy
  • STAR online Registration Information – Instructions on how to complete the online registration and submit the necessary documents.
  • Vancouver Coastal Health Prevention and Management of Occupational Exposure to Communicable Diseases Policy – you must provide your immunity status to the Post Graduate Dean’s office prior to commencing rotations at any VCH facility.
  • Vancouver Coastal Health Authority Legal Services & Information Privacy Office – you must complete a privacy and confidentiality learning module and print the certificate providing proof to your Program Director prior to commencing rotations.

To be submited to UBC:

  • Contract – You must sign and return the original to the Post Graduate Medical Education Office, keeping a copy for yourself
  • CAPER – Canadian Post-M.D. Education Registry, a national database used for creating statistics of post-M.D. training

To be submitted to Records and Benefits

  • Dues Deduction Form – complete this mandatory form and submit it to Records and Benefits
  • Direct Deposit Form – complete and submit to Records and Benefits
  • Healthcare Benefit Trust (HBT) Appointment of Beneficiary Card – complete this form to name the beneficiary of your life insurance policy. This is mandatory and should be sent to Records and Benefits
  • Healthcare Benefit Trust (HBT) Application for Group Benefits – complete this and submit it to Records and Benefits
  • Health Benefits Waiver Form – complete this only if you wish to decline any of the employer paid benefits available to you
  • MSP Form – Complete this form, attaching a copy of identification for each member to be covered and submit it to records and benefits
  • TD-1 and TD-1 BC – Complete these tax forms and submit them to Records and Benefits
